Vital Pulp Therapy (VPT) is a set of procedures aimed at maintaining the health and functionality of the dental pulp (the soft tissue inside the tooth that contains nerves and blood vessels) when it is exposed to injury or infection. This therapy is generally recommended for teeth that still have a healthy or reversible pulp condition but require intervention to prevent further damage, thereby allowing the tooth to remain vital (alive and functional). The goal of VPT is to preserve the tooth’s vitality and function, avoiding the need for more invasive treatments such as root canal therapy or tooth extraction.
Concepts of Vital Pulp Therapy:
Indications for VPT: VPT is typically indicated for teeth with a healthy pulp that has been exposed to trauma or decay but has not yet suffered irreversible damage. Some common scenarios where VPT is indicated include:
Dental caries: A deep cavity approaching the pulp.
Pulp exposure due to trauma: A fracture or a deep crack that exposes the pulp.
Recent dental procedures: A procedure like a cavity preparation or crown placement that inadvertently exposes the pulp.
Objectives:
To preserve the vitality of the pulp tissue.

Types of Vital Pulp Therapy:
Direct Pulp Capping: Direct pulp capping is a procedure performed when the pulp is directly exposed. In this technique, a biocompatible material (often calcium hydroxide or mineral trioxide aggregate, MTA) is applied directly over the exposed pulp to promote healing and prevent infection. This material stimulates the pulp to form a protective layer of dentin, known as tertiary or reparative dentin, which helps seal the pulp from further injury and bacterial contamination.
Procedure: The pulp exposure site is disinfected, and a biocompatible material is placed over the exposed pulp. A final restorative material, such as resin or amalgam, is then used to seal the cavity.
Success Factors: Direct pulp capping is generally more successful when the pulp exposure is small, and the pulp is healthy, showing no signs of infection or irreversible inflammation.
Indirect Pulp Capping: Indirect pulp capping is used when there is no direct exposure of the pulp but the decay is close to it. The procedure involves removing most of the decayed tissue, leaving a thin layer near the pulp to avoid exposure. A biocompatible material is placed over the remaining decayed tissue, and a final restorative material is placed over it. The goal is to allow the pulp to heal and form a protective layer without exposing it.
Procedure: The cavity is cleaned, and a layer of biocompatible material is applied to the remaining affected dentin, with a final restoration placed to seal the area.
Success Factors: Indirect pulp capping can be highly effective, especially when performed early in the decay process. It minimizes the risk of bacterial exposure to the pulp and can allow for natural remineralization of the dentin.
Pulpotomy: A pulpotomy is a procedure where part of the pulp tissue is removed, typically in cases where the decay or trauma has caused irreversible damage to the coronal part (top portion) of the pulp but the remaining pulp tissue is still healthy and vital. After the removal of the infected or damaged pulp tissue, a biocompatible material is placed over the remaining pulp to encourage healing. The tooth is then restored with a suitable filling.
Procedure: The coronal portion of the pulp is removed, and a biocompatible material is placed in the remaining pulp chamber. The tooth is then sealed with a restoration like a crown.
Success Factors: Pulpotomy is usually performed on primary (baby) teeth or permanent teeth with immature roots, where preserving the vitality of the tooth is especially important for future development.
Materials Used in Vital Pulp Therapy:
Calcium Hydroxide: A commonly used material for pulp capping due to its ability to stimulate the formation of reparative dentin.
Mineral Trioxide Aggregate (MTA): A more recent material that has gained popularity due to its excellent sealing properties, biocompatibility, and ability to promote pulp healing.
Bioceramics: These materials are used in modern pulp therapy due to their biocompatibility, longevity, and sealing ability.
Glass Ionomer Cement: Often used as a liner in pulp capping procedures, providing a barrier and promoting remineralization.
Benefits of Vital Pulp Therapy:

Pulp condition: The pulp should be free of infection or irreversible inflammation for the procedure to be successful.
Size of exposure: Smaller, controlled exposures generally result in higher success rates.
Time: Early intervention typically leads to better outcomes.
Material choice: The type of biocompatible material used plays a significant role in the healing process.
Follow-up care: Regular dental checkups are crucial to monitor the health of the tooth and ensure the pulp remains vital.
Complications and Limitations:
Although vital pulp therapy has a high success rate when performed correctly, there are potential risks and limitations:
Pulp necrosis: If bacteria infiltrate the pulp or if the pulp’s ability to heal is compromised, the pulp may die, requiring more extensive treatment.
Failure of capping material: If the restorative material does not adequately seal the tooth or if the pulp is exposed again, reinfection can occur.
Pain or discomfort: Some patients may experience mild to moderate pain post-procedure, which typically resolves after healing.
Vital pulp therapy is a conservative and effective treatment approach aimed at preserving the natural vitality of a tooth.
