Orthodontic treatment often involves the application of controlled forces to move teeth into their desired positions. However, the efficiency and direction of these movements depend heavily on anchorage — the resistance to unwanted tooth movement. Traditionally, anchorage has been achieved using intraoral structures (like other teeth) or extraoral devices (like headgear). However, these methods have limitations, including reliance on patient compliance and potential for undesired reciprocal movements. The advent of Temporary Anchorage Devices (TADs) has revolutionized orthodontic anchorage by offering a simple, reliable, and minimally invasive solution.
What Are TADs?
Temporary Anchorage Devices (TADs) are miniature titanium or stainless steel screws or plates that are temporarily fixed into the bone to serve as stable anchorage units. They are designed to be biocompatible, easy to insert and remove, and provide stationary anchorage during the course of orthodontic treatment.
Types of TADs:
Mini-Screws (Mini-implants):
Most commonly used type.
Inserted into alveolar or extra-alveolar bone.
Diameter: ~1.2–2 mm; Length: ~6–12 mm.
Mini-Plates:
Used in cases needing greater anchorage or when bone availability is limited.
Typically fixed with multiple screws and extend from more stable bony regions like the zygomatic buttress.
Onplants:
Placed under the periosteum and allowed to osseointegrate.
Rarely used today due to more convenient alternatives.
Common Sites of TAD Placement:
Maxilla:
Between premolars and molars (buccal cortex)
Palatal region (mid-palatal suture or paramedian)
Infrazygomatic crest
Mandible:
Between premolars and molars (buccal shelf area)
Retromolar pad area
The choice of site depends on the treatment goals, bone quality and quantity, soft tissue type, and patient-specific anatomical considerations.
Clinical Applications:
TADs expand the limits of conventional orthodontics by enabling:
Absolute anchorage (no movement of anchoring unit)
Intrusion or extrusion of teeth
Distalization or mesialization of molars
Correction of open bite or deep bite
Space closure without reciprocal anterior movement
Midline correction
Vertical control in skeletal discrepancies
Advantages of TADs:
Provide independent anchorage, unrelated to other teeth.
Minimize or eliminate the need for patient compliance.
Allow complex tooth movements without reciprocal effects.
Enable faster and more predictable treatment outcomes.
Are minimally invasive, easily placed and removed under local anesthesia.
Cost-effective compared to surgical alternatives or compliance-based appliances.
Limitations and Complications:
Failure of osseointegration or loosening, especially in areas with poor bone density.
Risk of soft tissue irritation or infection.
Root damage if improperly placed.
Requires careful case selection and radiographic planning.
Some patients may experience mild discomfort or anxiety.
Insertion and Removal:
Typically done under local anesthesia in a simple in-office procedure.
TADs can be immediately loaded in most cases, though light forces are recommended initially.
Removal is equally straightforward and typically leaves no significant residual defect.
Future Trends:
The development of digital planning, CBCT-guided placement, and 3D-printed surgical guides is enhancing the precision of TAD placement. Moreover, bioengineered surface modifications may improve osseointegration and stability. Research is also expanding into TAD-assisted clear aligner therapy, making them even more versatile.
