Management of Oro Antral Communication

Oroantral communication (OAC) is an abnormal connection between the oral cavity and the maxillary sinus, most commonly resulting from upper molar or premolar extractions. Prompt diagnosis and appropriate management are essential to prevent complications such as chronic sinusitis or oroantral fistula (OAF). Treatment depends on the size and duration of the defect. Small, fresh OACs may close spontaneously or with simple measures like a collagen plug and sutures, while larger or persistent defects often require surgical closure using techniques such as buccal advancement flaps, palatal flaps, or buccal fat pad grafts. Postoperative care includes sinus precautions and antibiotics to promote healing and prevent infection.
